Graduate of an accredited nursing school. Must possess a current license to practice nursing in the state of practice. Minimum of two (2) year of experience in general nursing practice, hospice experience and/or palliative, end of life care preferred. Should have supervisory and good communication skills. Ability to work well with others, function well under pressure, and function independently. Must comply with the standards outlined in the Nurse Practice Act.
REPORTS TO: Branch Administrator or Designee
PURPOSE OF THE POSITION:
The Nurse Liaison will handle admissions while also serving in a consultant role, acting as an advocate for the hospice program providing education for healthcare professionals in the market. The purpose of this position is to support the hospice patient and their family between the hospitals and community healthcare professionals to improve hospice care for all patients who are terminally ill and specifically to provide better management of hospice patients.
